Patient Portal lets you update important records and forms, verify personal information, view or confirm upcoming and past appointments, review medical information and securely message providers. You will also save valuable time, by accessing your information before arriving for your appointment.

To Register : Please send an email to SOD_PatientServices@mmc.edu to obtain your password.

Please include your Name and Date of Birth that was given at check-in. You must be an existing

patient in order to use the patient portal.
